• Registry Enrollment Criteria

    If patients meet one or more of the following criteria, they may be eligible to enroll in the registry. Patients may also want to consider having a risk assessment done for colon and rectal cancer if they have:

    • a personal or family history of colorectal cancer diagnosed at an unusually young age (under 50 years old)
    • a personal or family history of colonic polyps diagnosed at an unusually young age (under 45 years old) or in large numbers (more than 10 polyps)
    • belong to a family with an inherited colon cancer condition (i.e. Familial Adenomatous Polyposis (FAP) or Hereditary Nonpolyposis Colon Cancer (HNPCC) or Lynch Syndrome)
    • a personal or family history of certain extra-colonic cancers such as endometrial (lining of uterus) cancer (under 50 years old)

     

    An additional HNPCC Registry enrollment criterion includes having three family members, two of whom must be first-degree relatives, in two successive generations with this condition. These individuals must have colorectal cancer or colorectal cancer and a combination of gynecologic, genitourinary or other gastrointestinal cancer.

    It is helpful to collect other information before seeing a health care professional regarding hereditary colon and rectal cancer. Completing the Family History Form ahead of time is beneficial to your provider in evaluating your particular case. Listed below is specific information that you will want to collect:

    • People in your family who have developed cancer (any type) or other medical conditions such as gastrointestinal polyps.
    • How these individuals are related to you.
    • The age when they first developed the condition or other medical problems.
    • Their current age.
    • If they are deceased, their age at death and cause of death.

     

    It is also helpful if you provide us with your medical information and history at the time of your appointment. You may complete a Medical Release Form either beforehand or on the day of your appointment so that we may obtain your medical records from providers outside of Lahey Hospital & Medical Center.
