Urologic Cancers


Overview

Lahey has pioneered treatments for kidney, adrenal, bladder, prostate, and testicular cancers, offering every available option and allowing for the most appropriate care depending upon each patient's circumstances. Innovative therapies for superficial bladder cancer, bladder reconstruction, low-dose rate and high-dose rate brachytherapy (radioactive seed implantation) for prostate cancer, and an active research agenda are hallmarks of genitourinary cancer care at Lahey.

Lahey also provides minimally invasive surgical options for treatment of some genitourinary cancers, including laparoscopic prostatectomy and laparoscopic bladder reconstruction.

Adrenal Cancer

Treatment

  • Laparoscopic adrenalectomy: removal of the adrenal gland in patients with a malignant enlargement of the gland

Bladder Cancer

Screening

  • Cystoscopy: using a flexible instrument to view the patient's bladder
  • Cytology: testing for cancerous cells in the patient's urine

Treatment

  • Cystectomy: removal of the bladder
  • Laparoscopic Radical Cystectomy with Urinary Diversion: removal of the bladder in patients with bladder cancer and the creation of a bladder replacement; this includes the construction of a continent (i.e., neobladder, rectum-sigma-pouch) or incontinent (i.e., ileal loop) urinary diversion

Kidney Cancer

Treatment

  • Laparoscopic Partial Nephrectomy: technique used to remove a renal tumor as large as 4 to 5 cm, while preserving the kidney
  • Laparoscopic Radical Nephrectomy: removal of the whole kidney, including the Gerota's fascia and regional lymph nodes, in a tumor-bearing kidney with a tumor as large as 10 to 12 cm
  • Laparoscopic Nephroureterectomy: removal of the kidney, ureter and bladder cuff in cases of tumors of the renal pelvis and/or ureter

Prostate Cancer

Screening

  • Prostate-specific agent (PSA) blood test
  • Digital rectal exam

Diagnosis

  • Transrectal ultrasound used to obtain a biopsy and confirm presence of malignant cells

Clinical Center

  • Prostate Center

Treatment

  • Brachytherapy (radioactive seed implants)
  • External beam radiation therapy (EBRT)
  • High-dose rate brachytherapy
    • I125 brachytherapy
  • Hormone therapy
  • Laparoscopic pelvic lymph node dissection: removal of the regional lymph nodes of the prostate in patients with high risk prostate cancer
  • Laparoscopic radical prostatectomy: removal of the prostate gland in patients with prostate cancer; depending on individual circumstances, this procedure can be done with preservation of one or both of the nerves that are responsible for erectile function
  • Medical management (i.e., "watchful waiting")
  • Radiation therapy
  • Radical prostatectomy
  • Transurethral needle ablation (TUNA)

Testicular Cancer

Treatment

  • Laparoscopic Retroperitoneal Lymph Node Dissection (RPLND): the removal of retroperitoneal lymph nodes in patients with testicular cancer in clinical stage I or IIA/B, with avoidance of the large middle-line incision from the chest to the pubis
