Otherwise known as South American blastomycosis, this is an endemic disease produced by Paracoccidioides brasiliensis. The infection is found beginning at about 23 degrees North latitude in Mexico and extends southward to Argentina. Inhalation of conidia (fungal spores) is presumably the route of acquisition. Primary infection is asymptomatic in most cases, although the fungus can remain dormant for years within lymph nodes. The adult form, primarily affecting men over the age of 30, usually manifests with painful ulcerated lesions in the mouth. Other clinical presentations include cutaneous lesions, swollen lymph nodes, trouble swallowing and hoarseness. A clinical presentation identical to pulmonary tuberculosis may also occur.
