Lahey Clinic understands patients have the right to have their Medical wishes carried out in the event they are unable to make decisions themselves. The Healthcare Proxy helps you communicate your wishes by appointing a decision-maker (health care agent). Below are some suggestions, to assist and guide you through some items regarding this subject:

  • Choose a health care agent and an alternate agent
  • Read the "Health Care Decision Making" information (see below) at Lahey Clinic
  • Complete the Lahey Clinic's Health Care Proxy (two non-health care agent witnesses must be present)
  • Share your end-of-life wishes with your Health Care Agent and Alternate Health Care Agent
  • Give copies of the proxy to your agent, alternate agent, and doctors
