The purpose of this online application is to make it more convenient to request appointments at Lahey Clinic. However, please be aware that all requests received through the Online Appointment Center may take up to three full business days to process.

If you wish to request an appointment to see a physician within the next three days, or to cancel or reschedule an existing appointment that is scheduled to take place within three business days, call the Appointment Office directly at 781-744-8000, or toll-free (within the United States) at 800-524-3955.

We are only able to schedule Primary Care Provider appointments at Burlington and Peabody. All other sites will have to be called directly. For a list of other sites phone numbers, please visit the Other Key Telephone Numbers section of this web site.

Please note: For information regarding existing appointments, you must call the Appointment Office directly at 781-744-8000.

The data that will be gathered on the next several forms is the same information you would be asked for if you called the Appointment Office. This is an English-only application. After filling out the forms, you should expect a callback or appointment confirmation within three business days.

For your security, any information you provide will not be linked directly into the Lahey system. This means you may be asked for this information again at a later date.

It encrypts all your personal information-such as your Lahey Clinic number, your name, address, and reason for appointment-so that it cannot be read as the information travels over the Internet.

Please note that even though you may have registered certain demographic information by phone, in person, or by other (non-online) means, you will need to re-enter this data the first time you request an appointment online. However, the application will recognize you on all subsequent visits.
