By completing the following form, you can submit an online appointment request to see a Lahey Hospital & Medical Center physician. If you do not have a specific Lahey physician in mind, our staff will be happy to assist by scheduling an appointment with a provider who meets your needs.

Online appointment requests are processed in three business days. We are only able to schedule Primary Care Provider appointments at Burlington and Peabody. Please note: This form is not intended for urgent appointments or for appointments that you may need within three business days. If this is a medical emergency, immediately call 911 or go to the nearest emergency room.

Canceling or Rescheduling Existing Appointments

To cancel or reschedule an appointment scheduled to take place within the next three business days, please call the Appointment Office at either of the numbers provided above.
